What is the best way to divide partitions?

I'm using KLOXO-MR since 2014.
My server is working well and I appreciate Mustafa for limitless support.
So I decided to add another server.
This time I will install pure kloxo-mr on minimal Centos 6.8 from the start.

Here is specifications of my server.

i3-4150
16G RAM
SSD 256G
HDD 2T

To be honest, I have a little knowledge of server management.
So I wonder what's the best way to divide partitions.

At first time I tried to divide ssd to 3 partitions(/boot, /swap-16G. / ) and hdd to 2 partition (/backup, /home).
But in kloxo-mr, the backup files for clients are located in /home/kloxo/client.
So I think that the /home partition shoud be located at hdd (the bigger).
Am I right?

If not please tell me how to divide partitions to make better performance.

I am always use:
 /boot -> 256-512MB
/ (root) -> rest of HDD space
SWAP -> 4GB

Thank you for reply.

It seems a case of using only one storage.

But I use two storages(256G SSD + 2T HDD)
So can I divide like this?

SSD
/swap -> 4G
/boot -> 512MB
/ -> rest of SSD

HDD
/home -> whole
