Why always appear "****SPAM ***" prefix in mail titles ?

Hello everyone. Today 's a nice day 
When i use a standalone server for all service, mail server work perfectly. Then, i have a cluster:
10.0.0.249 kloxo portal,
10.0.0.248 web server + mail server.
And i have this problem. Please see my attached picture.

Go to 'Spam Status' and reduce the value of 'Score At Which Mail Is Judged Spam'. If this issue appear for your email account, read http://forum.mratwork.com/kloxo-mr-tips-and-tricks/(tip)-how-to-setup-qmail-on-kloxo-mr/ because still something wrong with hostname, spf and or rdns.

I just demo it on localhost I set the value of 'Score At Which Mail Is Judged Spam' = 8 and it work very nice.
