menu tree missing icons

I'm new here so forgive any ignorance.  I recently switched over to your kloxo-mr from kloxo after the whole security hack issue.  I am running your control panel on 3 servers 2 with 1 company and the third with a different company.  On the solo server everything looks correctly.  at the dual server location the menu looks like the attached image.  All three servers are running Centos 6 32bit.  I'm assuming that the difference has something to do with the Centos image used to load the server.  Have you seen this before and do you know what I can do to remedy it.

Just running 'yum reinstall kloxomr; sh /script/cleanup'.

Just running 'yum reinstall kloxomr; sh /script/cleanup'.
So run "yum reinstall kloxomr"  And then do a "sh /script/cleanup"?  OR is that one command line and not 2

Try clear browser cache and enable simple skin and then disable simple skin.
